If you are new to the ECW health insurance plan you should be sure and read your summary of benefits booklet. You should have received this booklet when you enrolled.

ECW has contracted with Corporate Benefit Services of America, Inc. to administer the ECW health insurance plan. They are the ones that receive the bills from your health provider, review the claims and pay the invoices if approved.

CBSA has a responsibility to ECW to promptly, but accurately process all claims. Occassionally though they require further information from either you or the provider. This is done to insure that your rates don't go up because of items such as double billing.

ECW also contracts with Allord Reed and Company, of Madison, Wisconsin. Should your local benefits administrator, and CBSA claims be unable to answer your questions please feel free to call them at 1-800-434-0066.

You can also speak with your local cooperative general manager if you feel that something is not being handled correctly. Each member cooperative general manager that subscribes to the ECW health plan is a member of the board of trustees.

After enrolling you should have received a subscriber card. The subscriber card is the only card you need to have your claim processed. While most health care providers will bill directly, they are not required to do so. Should they not accept your card or be willing to bill the insurance company directly, you may be responsible to pay the bill and then submit your bill via a claim form. See your local benefit administrator if this is the case. The following information is located on your FRONT of your subscriber card:

  • Your Name
  • Your Social Security Number
  • The effective date of your coverage
  • Your Group Number
  • Your Group Name (Same as employer)
  • The type of coverage you have
  • The amount of any Medical COPAY required
  • The Prescription Drug Plan Administrator
  • The Prescription Drug Group Number (RX Group)
  • The phone number for Drug Card Help
  • The amount of any COPAY required for prescriptions
  • The name of your PPO Network is in the upper right corner.

Located on the BACK of your subscriber card you will find:

  • Address of where to submit PPO Claims
  • Address of where to submit other claims
  • The benefit/claim customer service phone number
  • Electronic claims information
  • 24 Hour Automated Customer Service Phone Number
  • Pre-Certification phone number
  • Discounted Eyewear Membership Logo
